Interior Health is seeking an experienced and registered Social Worker for a relief part time (0.71 FTE) opportunity to join our close-knit multidisciplinary team in the outdoor paradise of Revelstoke, BC. The community care social worker is responsible for the frail elderly, patients with dementia and is also the designated response for adult guardianship. The social worker works as sole charge and must be capable of working independently to evaluate high-risk situations and mitigate risk with minimal supervision. A thorough understanding of adult guardianship legislation, the Mental Health Act, Public Guardian Act and any policies around disability pensions is a must-have for this position. The Community Care portion of this position will be in the City of Revelstoke with exception of some minimal travel within the geography catchment area. Catchment area would be Trout Lake, Mica Creek, and Malakwa.
The Long-term Care portion of this position will be based out of Mount Cartier Court, located next to Queen Victoria Hospital on a beautiful private lot which backs on to a forest. The facility was built in 2005 based on a cottage-style model. This facility has 55 private rooms over 3 distinct cottages that are complete with their own kitchen, dining area, sitting area and courtyard. The care philosophy is gentle care in a home-like environment.
This is a Sole Charge position and is the only Interior Health Social Worker within this community. The successful candidate would be connected in with the Interior Health Social Worker Professional Practice Council of the North Okanagan area and the wider reaches of Interior Health Social Work.
In this position with Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.
Typical Duties and Responsibilities:
• Provides direct social work services and care coordination to individuals, families and groups.
• Provides a social work perspective to team practices; participates in planning and program development activities; and provides operational, policy and ethical practice recommendations through reporting, consultation and collection of information on specific client groups and/or clinical areas.
• Maintains client records.
• As required, takes leadership or co-leadership role in client/family or Collaborative Care Plan conferences.
• Acts as a resource person to the multidisciplinary/program teams and other health professionals in the hospital, community, and health service area.
• Maintains basic knowledge of developments in the Social Work profession and sets measurable goals and objectives within a team or individual environment.
Shifts of work are Tuesday to Friday 08:00 to 16:00 and 08:30 to 12:15. This relief position is scheduled to end February 23, 2021.

Bachelor’s Degree in Social Work from an accredited university.
Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers.
Current valid BC driver’s license.
**Two years’ experience in a public health setting in the community is preferred.
**Knowledge of Adult Guardianship legislation, Mental Health Act, Public Guardian Act is an asset.
